A mother has been accused of murdering her screaming 20-month-old foster son while her terrified children locked themselves in a room in fear.

The woman, 41, who cannot be named for legal reasons, appeared in Dubbo Local Court, central New South Wales, on Thursday charged with one count of murder.

The court heard the child, who could not be revived on March 23, 2015, suffered blunt force trauma to his head, a broken femur, a penetrative injury to his bottom and was found with vomit in his mouth.

The court heard the boy likely died from the trauma to his head but the broken leg was compared to an injury sustained in a high-speed car crash by a pathologist, 7NEWS reported.

A trauma specialist described the injury to the child’s bottom as ‘forceful’.

Police will allege the mother, who has three biological children, was seen pushing the foster son into the bath on the night of his death.

According to court documents, she then told her daughter to not to tell investigators about the earlier bathtub incident.

The woman’s daughter was so fearful about her mother’s temper she had locked herself in a room with another sibling.

The foster mother has given a number of varying accounts about the evening of the child’s death, which happened at the family’s home 100 kilometres from Dubbo.

A police recording with the mother and her partner, she said she ‘lack(ed) clear memory about the event’, according to court documents.

The boy’s biological mother was ‘left in the dark’ when the foster mother was arrested for the alleged murder.

She had given the child to Family and Community Services, who gave the child to foster care provider Uniting to find a guardian.

The biological mother, who died in January 2019, gave the child to the service until she could free herself from a domestic violence situation.

The foster mother was keen to have the child for a long-term placement but became angry in the weeks before his death when she learnt it was likely to be a shorter stay.

The mother, who is yet to plead, will return for her hearing before Magistrate Lisa Stapleton in July.
